- Previous interventions haven't produced the expected improvement.
- You want a second, specialist read before changing course.
- A case would benefit from formal testing (MMPI-3, MCMI-IV, Wechsler).

Assessment reports include recommendations for the patient and for you — so you can keep treating with added insight when that's the right call.
